RSVP FOR UNDERGRADUATE BIOLOGY GRADUATION RECOGNITION CEREMONY 2017

DEADLINE to be included in the program: Monday, May 1, 2017.

Yes! I plan to attend the 2017 KU Undergraduate Biology Program and Human Biology Program graduation recognition ceremony and reception on Sat., May 13, 2017, at 1:30 p.m. in Crafton-Preyer Theatre, Murphy Hall.

Name as you would like it to be printed in the program (example: Ashley Jane Clough): *

Name as you would like it to be read during the ceremony (example: Ashley Clough): *

If your first or last name might be difficult to pronounce, please write it phonetically (example: Cluff):

I can be contacted at:

List any non-biology majors and minors if you would like:

Special accomplishments (list in priority order—program space is limited; wording will be altered for consistency throughout the program) to be included in the printed program (example: graduation with honors, participation in a research project [name the professor], academic award, officer in student government, etc.) Please do not feel that you must have special accomplishments—an undergraduate degree in and of itself is deserving of recognition!

(limit 500 characters, with spaces)

Plans after graduation to be included in the printed program (example: graduate school for master’s or Ph.D., medical school, position with a particular company or agency, etc.) Although this is optional we really are interested in what our graduates intend to do.

(limit 250 characters, with spaces)

My nomination for "Favorite Biology Professor"—Please limit your nomination to a faculty member teaching a 300-level or above biology course. Briefly describe why this faculty member is your favorite. Dr. Stephen Benedict was chosen in 2016 and so is not eligible for the award this year.

We need your photo …

Before the ceremony begins, we show a slideshow of photos of as many graduating seniors as possible. Please upload a photo of yourself. If you do not have a digital photo, you can scan prints in the Biology Teaching Resource Center, 1004 Haworth.

Accepted file types: .jpg, .gif, .bmp, .png
Files must be less than 2 MB.
Allowed file types: gif jpg png bmp.

Want to thank your parents, a faculty member, your friends? We can include a short message (15 words or fewer) to accompany your photo.

If you are in need of an accommodation due to a disability, we ask that you submit your request either in this web form or via email to Jen Weghorst, Asst. Director, (weghorst@ku.edu; Phone: 785-864-5883, TTY: 711) on or before Mon., May 1.
